2012 Online Nomination Form

San Diego Junior Theatre annually presents the Junior Theatre Honors in the following two categories:

Alumni Achievement: Presented to San Diego Junior Theatre alumni in recognition of exemplary lifetime achievement in their careers, professions, occupations, or community service. Junior Theatre alumni eligible for this award are individuals who had a meaningful relationship and experience with San Diego Junior Theatre through participation in (i) shows or productions as a cast or crew member, (ii) classes, camps, workshops, or community collaborations/partnerships, and/or (iii) Confetti.

Distinguished Service: Presented to individuals in recognition of their distinguished or extraordinary service to San Diego Junior Theatre, as exhibited by the giving of their time, talent or treasure to Junior Theatre. It is not necessary to be a Junior Theatre alumnus to receive a Junior Theatre Honor in this category.

Current members of (i) the San Diego Junior Theatre staff or faculty, (ii) the Board of Trustees of San Diego Junior Theatre, and (iii) the committees of the Board of Trustees, including the Junior Theatre Honors Recognition Committee, are not eligible to receive the Junior Theatre Honors. Honorees are expected to attend the Junior Theatre Honors award presentation event to be held at Casa del Prado Theatre in Balboa Park, San Diego, California, on November 17, 2012, as a condition to receiving the award.

To assist in the selection of the Junior Theatre Honors recipients, you are strongly encouraged to submit with this Nomination Form collateral materials and information that support your nomination and that describe why the nominee should be recognized with a Junior Theatre Honor in the category you indicated. Such materials and information may include the nominee’s resume, letters of recommendation from other individuals as to why the nominee should receive this award, news articles, and other awards or accolades the nominee has received.
